How to fill in the dedication list

Note:
1. Lamps can be offered to Shakyamuni Buddha, Amitabha Buddha, Medicine Buddha, Avalokiteshvara Bodhisattva, and Manjushri Bodhisattva; the names engraved on the lamps should not exceed five characters. If lamps have already been lit for renewal or dedicated to the deceased, please specify this.
Example: 10. Offering lamps of light to Shakyamuni Buddha, dedicating the merit to XXX, the deceased, and for their continued life.
2. The yellow memorial tablet is set up for the deceased, so please also note the name of the person who set up the tablet.
Example: 7. Small yellow memorial tablet, dedicated to XX's ancestors, living ancestors: XX
