How to fill in the dedication list
Note:
1. Lamps can be offered to Shakyamuni Buddha, Amitabha Buddha, Medicine Buddha, Avalokiteshvara Bodhisattva, and Manjushri Bodhisattva; the names engraved on the lamps should not exceed five characters. If the lamps have already been renewed or dedicated to the deceased, please specify this.
Example: 10. Offering lamps of light to Shakyamuni Buddha, dedicating the merit to XXX, the deceased, and the continuation of their lives.
*If you need multiple memorial tablets, add them to the offering list and then return to fill in the duplicate entries.
